三：设计及制度缺陷导致的重大隐患：

5	装置未经过正规设计且未完成设计诊断。	编者注：HAZOP、MOC等常用的化工企业安全管理工具进入强制性要求。
6	新开发的生产工艺未经HAZOP分析、工业化试验以及省级人民政府有关部门组织的安全可靠性论证，直接进行工业化生产。
7	未建立或未严格执行变更管理制度。
8	未建立安全（工艺）操作规程和工艺控制指标。
21	未将企业涉及的危险化学品信息收集整理齐全，并根据其危险特性制定安全操作规定。
40	危险化学品未按照分区分类存放标准储存，或超量、超品种储存危险化学品，或相互禁配物质混放混存。
